Water connection questions

1. Would you like to take advantage of this opportunity to connect to the water system at no cost? Options: Yes or No. If yes, continue to the remaining questions. If no, sign and return the form.
2. Does your property have a house or structure with existing well water service? Options: Yes or No. If no, choose one of these options: freeze-proof yard hydrant installed with water service; water line run to a location up to 20 feet from the meter, capped and marked with a stake; or no service installed at this time.
3. For properties with a structure and existing well water, do you want to connect to your existing water service line? Options: Yes or No. If yes, indicate whether you have located it within 5 feet of the structure and marked its location. If no, indicate whether you want to connect water service to a new water line that you will install at a new location.
4. If you are moving your water service line to a new location, have you upgraded the service line from inside of the structure to at least 18 inches outside of the structure? Options: Yes; No; Not yet, but I will before my connection day.
5. To the best of your knowledge, is your water system ready for the connection? Options: Yes; No; Unsure. If no, list the date when it would be ready.
6. Leave any comments, questions, or concerns with the water connections or project work that has been done so far.
